Appointments and Hours

If you are a new patient to Northwoods Urology Associates and would like to make an appointment, please call the location of your choice from the list at the bottom of the page. For the convenience of our patients five office locations are offered. Office hours are Monday through Thursday from 8 a.m. to 5 p.m. and Fridays from 8 a.m. to 1 p.m. Twenty-four hour emergency services are available during the hours the office is not opened.

If you are a new or existing patient, please be sure to bring the following items to your next appointment:

• Identification
• Insurance card
• Referrals (if required by your insurance company)
• Medical or laboratory tests
• A list of medications that you are currently taking
• Any co-payments that are required at the time of your visit (if applicable)
