It has been a great pleasure to serve you over the past year. I am writing to announce that I will be leaving Northwoods Urology of Texas and will no longer see patients here after September 2024. I will be joining Memorial Hermann Medical Group in Katy starting in October 2024.
You may continue your care at Northwoods with any one of our other providers. Your records will remain confidential and available to you at Northwoods Urology of Texas. You may obtain a copy of your medical records by submitting a request with proper authorization to Northwoods Urology of Texas. fI you wish to have your records transferred to me or a different physician outside of Northwoods, you are entitled to do so, again with proper authorization.
