Please bring your completed paperwork with you at the time of the appointment.

Q: I need to see a Urologist, do I need a referral?
A: When scheduling your appointment we will inform you if you need to bring in a referral form. Many insurance providers make it mandatory for patients to receive a referral from their primary care physician in order to see a specialist, such as a Urologist. If this is the case, it is the patients responsibility to contact their primary care physician and request the physician fax over a referral form to us at 936.273.6911. This way we can have the form in our system prior to your appointment and you do not have to keep up with this form until the day of your appointment.
Q: What do I need to bring with me?
A: To save on time please bring in your completed paperwork, a recent photo ID / driver’s license, insurance card, questions & a list of medications that you currently take. If you have any CT or ultrasound images please bring these with you. Any form of medical records or lab results that you can share with your Urologist will speed up the process. If the Urologist is needing any image and does not have the correct information at hand at the time of your appointment this will cause delays. Please follow up with your primary care physician prior to your appointment and have them fax over all necessary reports and images to 936.273.6911.
Q: Should I come prepared to give a urine sample?
A: It’s always a great idea to be able to provide a urine sample on the spot when you visit your Urologist. The majority of our appointments will need some form of a urine analysis. Being prepared to give a sample is always the best.
