If you are a new patient to Northwoods Urology and would like to make an appointment, please call the location of your choice. Office hours are Monday through Thursday from 8 a.m. to 5 p.m. and Fridays from 8 a.m. to 1 p.m. Twenty-four hour emergency services are available during the hours the office is not opened.
If you are a new or existing patient, please be sure to bring the following items to your next appointment:
- Identification
- Insurance card
- Referrals (if required by your insurance company)
- Medical or laboratory tests
- A list of medications that you are currently taking
- Any co-payments that are required at the time of your visit (if applicable)
